It refers to any form of media that is either not appropriate for young ages, offensive, vulgar, profane, crude, etc...
Personally, I don't see any value in creating media that attracts the attention of dangerous and malicious viewers. I'm not going to tell you that you shouldn't, I am only warning you that the results of NSFW media may lead to some unwelcome online interactions.

I don't think blood counts as NSFW specifically, but rather mature content. That is to say if it implies acts of vio.lence, relative to horror/macabre, the level of maturity is relative from there.
