He isn't.
So most fear him. Some have even tried to fight him, but Aurora just picked them up and dropped them off at the hospital lobby.
Kiki sees him in a different way, she thinks he's just a demon like being who keeps little tiny horse toys in his office drawers.
And she loves him.
