From what I can tell from seeing creepy art styles, most of it comes from obscured features (such as only the eyes and mouth being visible, eyes not visible, etc.) and/or lots of darkness in the environment. After all, sometimes the scariest thing is the unknown.
I guess you could also go the grotesque route with more disgusting features, but it's a lot more work. It really works tho, and can definetly be used in combination with obscurity.
