When a Dreamer dies their soul goes up to the heavens and they become a star, to watch and guide the ones that they love who are still living.

The Dreamer light there heart reflection is what become the star, glowing with a slight color variants to them.

When a Nightmare is Born it's said a shooting star can be seen somewhere.

Someone who has special eyes or know what to look for ( like Omen ) can tell normal Stars from a Dreamer heart, we're the star fall to or more likely draw to is unknown...

a few clues were mentioned in Skey when Dreamcatcher was struggling with his nightmare syndrome, Victoria told him two things. "Don't give into the Void" and "Don't fall into the abyss "

At the time this all happened I was a pretty bad writer and could never describe exactly what I was wanting for the scene with Catch struggle against Catty how I really wanted to...

I'll describe how it would be like in a Dreamers mind
When they first gets nightmare syndrome, 
Their mind is full of memories, their hopes their dreams, all the things that happened up to this point in time but unknown to them a small black Bloch has surfaced in this world, it's painless and small, completely unnoticed, this Bloch starts absorbing the memories close to it, eating what it wasn't and 'discarding' what it doesn't... As it grows it replaced eatin areas with copy of what it stole, so it's harder for the Dreamer to realize what happened to themselves.

As the curse grows the Dreamer will experience more physical symptoms of nightmare syndrome but so will there mind, Dreamer will suffer memory loss from part of them being discarded and as more of the Dreamer mind has been replaced with fake copies the they will lose a footing I'm there own mind as the Nightmare slowly grains more ground.

As the nightmare syndrome reaches its most critical stages and the Dreamer is past the point of no return, the Dreamer will try and hold on to their most dearest in precious memories they have left...

But the best way to describe a Dreamer last moment in their minds would be something like this...

A black vortex devouring all that is left of the poor Dreamer memory, shredding them apart and sucking them into the abyss....and with the last light of the Dreamer gone the Nightmare is Born....

A little bonus fun fact: Catch when he was just a fawn had bad dreams of falling and then broke his arm and got a fear of heights... This is all intertwined with a Dreamer as a star falling from the sky

I have made comment before about a light Eater discarding what they don't need of the Dreamers heart...
But the question is what happened to that part?...
