Little Ford might somehow know what plastic surgery is- but he IS still just a child, so he doesn't entirely understand what the word "hobby" means.
He thinks the meaning of it is something you do a lot, which is slightly correct, but he doesn't realize that it also means that you enjoy doing a hobby, so he mentioned how he has to do the clean around the house often since his mother is lazy.
