He basically was travelling in the Time Realm where time is frozen. When you travel back, you cross between frozen time and moving time. As he was heading back he got rekt by a Realm Guardian. The wound it dealt is permanent bc part of it is frozen and time, but hurts him bc he is now in the normal world where time actually moves. Since its 'frozen', its unable to kill him
