The final boss of the first #Touhou game's Hell route, #Konngara
Funnily enough, Konngara is far more merciful than the boss of the Makai route, Sariel.
Like many of the characters in the first game, not much is known about her (or him, with both of Reiiden's final bosses, it's hard to tell). All we know is that she has a sword and and a thing sticking out of her head which may or may not be a horn.

Thanks! Yeah, the PC-98 games are already fairly obscure, but the first game is even more so-- probably because it's more like Arkanoid mixed with some bullet hell elements, rather than being a bullet hell shoot-'em-up.
Reiiden is a strange beast, to be sure, but it has a really great soundtrack. I highly recommend giving it a listen.
