re: The bully knows she's being a bully. It has nothing to do with jokes or being funny. If it did, the moment she knew it wasn't funny, she'd do something else. Instead, it's really about teasing the girl and putting her down so the bully feels superior. Which means, the bully doesn't feel superior but is insecure inside. If she felt great about herself, she wouldn't need to put someone down to lift herself up by comparison. Confronting her with the fact she hates herself might make her defensive and she might verbally attack you. You could try to make her understand how her words affect others by having her imagine what if it were her. You could also tell others what's going on, so they can also tell her to stop and it won't just be you. I'm sure others feel bad and just need encouragement to speak up. If everyone calls her out on her abuse, she won't ge able to continue.
If having yourself and others tell her off doesn't work, you also have the option of having yourself and others try ignoring her. Personally, I'd like to see if she can be reasoned with and talked some sense into, but not every bully can be changed. Some can, so it's worth trying. But, no matter whether you do it alone or with the help of others, you can't just deal with the bully. The girl needs to know that the things she says to her aren't true. They may be hurtful, but lies, even disguised as "jokes", can't change who a person really is inside. Help her see what kind of a person she is to grow her confidence. People won't always be able to help her, so helping to build her confidence will help her be able to deal with things herself too. School councilors can also help, but friends are preferable.
