She's not a unicorn lady. She's an oni-or at least we in the Touhou fandom are pretty sure she is. 98 percent sure. ZUN doesn't really tell us much about PC98 characters nowadays. (We cry out for Mima to return, but he doesn't care! TT-TT)
For all we know, the object sticking out of Konngara's head could be a spike that was driven in there. It does seem that there's a drop or two of blood at the base of her "horn".
