The "Bring Your Cat to Work Day" Insident:
GLaDOS was turned on and requested a cat, a cardboard box, and some neurotoxin. She then flooded Aperture and forced the workers to go through tests, with a promised reward of cake. Instead of cake, your either went to in incinerator (or Android Hell) or you got trapped in a room filled with neurotoxin. This is where her famos quote "It only requires a little neurotoxin" comes from.
